Family Forum Inc. Project Head Start is accepting applications and determining enrollment for their quality early childhood program for prenatal women and children birth to five years of age.
Applications can be received by either stopping by the main office at 2231 Catlin Ave., phone 392-9896 or at the Head Start sites, Center I at 1500 N. 34th St., Suite 400, phone 392-6286 or Center 2 at 518 Grand Ave., phone 392-9848.
Parents or guardians must provide proof of income such as W-2 forms, income tax forms or present paycheck stubs when applying, as income eligibility is required.
Head Start requires a physical exam, dental exam and a record of immunizations.
