Brief statement of dates and facts: (If this Is an eviction action and you are seeking money damages, you must also state that claim on this form,) On March 5, 2022, Michael Mackey was operntlng his 2004 Chevl’olet Suburban on E, 2nd Street in the City of Superior, Douglas County, Wisconsin when the vehicle left the roadway and struck a light pole owned by the City of Superior, causing damage which cost $2,358.62 to repair, See the attached Benson Electric Company invoice and the police incident reports. Due to Macky’s negligent use of his vehicle the City has suffered damages in repairing the light pole, Mackey did not carry a policy of liability insurance on the vehicle. /s/ Harley L.
