St. Francis in the Park Health & Rehabilitation is joining the Improving Transitions of Care initiative in Duluth.
This initiative is a Medicare Quality Improvement Initiative where Stratis Health partners with health care communities locally to improve care for patients during transition from one health care setting to another.
The initiative consists of representatives from local acute care hospitals, clinics, home health and hospice organizations, long-term care facilities, assisted living facilities, public health departments, patients or patient advocates and other pertinent community partners.
The goal of this initiative is to reduce the number of avoidable hospital readmissions, which helps reduce Medicare costs and improves overall care for patients.
